Physicians who meet all of the requirements of the Interstate Medical Licensure Compact receive a formal Letter of Qualification (LOQ) from their State of Principal License, indicating that they may participate in the Compact. Physicians then may select the states within the Compact where they wish to be licensed. These states, upon receiving the physician’s formal Letter of Qualification, issue licenses.

Once your LOQ has been issued, you may use it for 365 days to obtain licenses from multiple states through the Compact. At any time during the 365-day period when a Letter of Qualification is valid, you can return to this website and request additional Compact member states for licensure.

At the end of this period, you must reapply for a new LOQ in order to continue using it to obtain new licenses in multiple states.  Applying for a second or subsequent LOQ is relatively simple.

If you do not wish to obtain any new licenses, you don’t have to apply for a new LOQ.

A background check is required for all Letter of Qualification applications; however, fingerprint requirements vary from state to state. If required, the fingerprinting information will be sent to you by your State of Principal Licensure, and not the IMLCC.

Please check with your State of Principal Licensure if you have any questions.

Physicians are responsible for knowing the start date of their LOQ.

NOTE - Letters of Qualification are only valid for 365 days from the date of issuance.

Please be advised that this is a non-refundable application process. The application fee is $700. (You will also be required later – if you qualify for licensure through the Compact – to pay the licensing fee of each state in which you would like to practice. Visit our list of state fees to learn more about these costs.)

Thank you in advance for your cooperation. You may now begin the LOQ reapplication process by clicking on the “Go to Reapply for Letter of Qualification” button below.
